Accidental Death & Dismemberment Insurance
:: Plan Description

An unexpected death or disabling injury can be an emotional and financial burden on you and your family. Protecting those you love is important, so we've arranged to make available to you an Accidental Death & Dismemberment Insurance Program designed to help protect your family's financial security in the event of a covered accident.

Our Gift To You
As a Credit Union member, age 18 to 74, you are eligible for up to $2,000 of Basic Accidental Death & Dismemberment Insurance provided at no cost to you. This coverage is fully paid for by the Credit Union! This plan offers 24-hour protection against a covered accident anywhere in the world, on or off the job, on business, on vacation or at home.

Additional Coverage For You and Your Family - Up To $300,000
We have also made arrangements to make up to $300,000 of Additional Coverage available to all Credit Union members, age 18 to 74, at affordable group rates. Plus, when you purchase Additional Coverage, it will include the following benefits:

1. Family Coverage
If Family Coverage is selected, your spouse will automatically be insured for 50% of your benefit (60% if there are no dependent children). All dependent children under age 19; 26 if in UT (age 23 if the child is a full-time student in an accredited institution of higher education) will be covered for 20% of your benefit (25% if you do not have a spouse). In the event of your death, Family Coverage may continue provided your spouse remains covered.

2. Common Carrier Coverage
Benefits will be doubled if the covered loss results from a death that occurs while you are riding as a fare-paying passenger, in or on a licensed public conveyance being operated commercially by a common carrier for the regular transport of passengers.

3. Education Benefit
(Not available in MI.) The policy will pay in addition to all other benefits, 2% annually of your Additional Coverage in force (up to a maximum of $6,000 per year for a maximum of four years) on behalf of any dependent child who, on the date of the accident, was enrolled as a full-time student in any school for higher learning beyond the 12th grade level. If no children qualify, the death benefit is increased by 2%.

4. 'Cost of Living' Benefit (Accumulation Protection)
Your Additional Coverage will increase 5% after 24 months of continuous coverage and every 24 months thereafter for 10 years until the amount of insurance is equal to 125% of the original amount of insurance; with no increase in premium.

5. Accident In-Hospital Benefit
When a covered accident requires that you be hospitalized, the plan will pay a monthly benefit of 1% of your Additional Coverage beginning with the first day of hospitalization so long as you are hospitalized for at least 7 continuous days (maximum of up to $3,000 per month for a period of up to 12 months).

6. Return of Premium Under this benefit, all premiums paid for coverage will be returned to the beneficiary if you should die do to any cause, natural or accidental (excluding suicide).

Note: Benefits 1-6 described above are not included with $2,000 Basic Insurance Coverage.

*Note: Coverage reduces at later ages, as described below.Your Accidental Death Insurance coverage will remain in effect as long as the Master Policy remains in effect, you remain a member of the credit union and your premiums are paid when due. Premium rates may be changed on any premium due date. Any premium increase will apply to the entire master group policy. We will provide written notice 31 days prior to any change.

Only one amount, the largest to which the Insured Person is entitled, is paid for all losses resulting from one accident.

Effective Date of Coverage
The $2,000 of Basic Coverage is effective the first of the month following acceptance of the Enrollment Form. Any Additional Coverage you may have purchased is effective upon acceptance of the Enrollment Form and the first premium payment. A Certificate of Insurance will be provided to you.

Limitations and Reductions
Once enrolled, benefits are available regardless of age. The amount of insurance paid to your beneficiary depends on your age at the time of your death or injury. The full face amount of your $2,000 Basic Coverage and your optional Additional Coverage will be paid if you are under age 70, 50% of the face amount if you are from age 70 to 74 and 25% at age 75 and over. You may have up to $1,000,000 of accidental death or accidental death and dismemberment coverage in effect with us or any Aegon affiliate at any one time. All premiums paid for anything in excess of $1,000,000 will be refunded and the excess voided. Our aggregate limit of liability for all benefits payable for all Covered Persons as a result of a common accident is $1,000,000.

Your coverage will end on the first of the following dates: (1) the premium due date you fail to pay the required premium except as provided in the Grace Period (not applicable to $2,000 basic-only coverage); (2) the date the Policy is terminated; (3) the premium due date after you are no longer a member of the Credit Union; or (4) the premium due date next following the date the Credit Union ceases to participate in this plan of insurance.

We will not pay a benefit for a loss which is caused by, results from, or contributed to by: (1) suicide, attempted suicide or intentionally self-inflicted injury, while sane or insane (in MO and CO while sane); (2) declared or undeclared war or any act of war; (3) full-time military service; (4) participating in a riot; committing an assault or felony; (5) Sickness or its medical or surgical treatment, including diagnosis; (6) bacterial infection except through a wound accidentally sustained; (7) operating or riding in any kind of aircraft except as a fare-paying passenger on a regularly scheduled commercial flight or as a passenger in a transport plane operated by the Air Mobility Command (AMC) of the United States of America; (8) alcohol intoxication as defined in the state where the accident causing the injury occurred; (9) taking of any drug, medication, narcotic or hallucinogen unless as prescribed by a Physician; (10) taking of alcohol in combination with any drug, medication or sedative; (11) voluntary gas inhalation or poison voluntarily taken, administered or inhaled; (12) riding or driving as a professional in any kind of race for prize money or profit. Exclusions may differ in your state. See your Certificate of Insurance for details.

30 Day Money Back Review Period
You have the right to examine your Certificate of Insurance without obligation. If, within 30 days of receiving your Certificate, you are not completely satisfied, simply write "cancel" on the Certificate and return it by mail. Any premiums you may have paid will be fully and promptly refunded.
